Transcultural nursing

Transcultural nursing focuses on different cultures and religious clients.



Acculturation is a process in which an individual adopts, acquires, and adjusts to a new cultural environment.


What is a basic way to assess for cultural or religious differences in a client?

Ask the client if there are any practices or food preferences to be aware of.

These preferences can alter a client's health outcomes and should be incorporated whenever possible.


If a cultural practice seems strange, how should the nurse respond?

Allow the client to continue the practice as long as it doesn't cause harm.


What is cupping therapy?

Cupping therapy is an ancient form of alternative medicine in which a therapist puts special cups on the skin for a few minutes to create suction.

Clients get it for many purposes, including to help with pain, inflammation, blood flow, relaxation and well-being, and as a type of deep-tissue massage.


When a client nods yes, does it always mean yes?

Nodding yes may NOT aways mean yes.


What is typical appropriate eye contact and physical closeness when speaking with clients?

Most clients don't like constant eye contact or getting too close to them during conversations.

Be sure to look away at times and stand at least a few feet away from client.


Regardless of ethnic background, what diseases are clients most at risk for when living in North America?

Clients living in North America are higher risk for:

  • hypertension
  • heart disease
  • cancer
  • diabetes mellitus
  • obesity

The SAD (standard American diet) diet is high in saturated fat and a sedentary lifestyle is more common in North America.


What substance is frequently prohibited in many religions?



What should be done if a client does not speak the same language as the HCP or nurse?

With language barriers:

  • get an interpreter
  • provide written instructions in the client's language


What is done if the client refuses to be cared for by someone of the opposite sex?

Try to accommodate the client and get a same sex caretaker.


Is it OK to use humor when communicating with clients, especially those of a different background?

Avoid using humor since it can be misunderstood as disrespectful.


What conditions are African Americans most at risk for?

African Americans are more at risk for:

  • sickle cell anemia
  • lactose intolerance

  • may not divulge personal information at first


What are some cultural considerations for the Amish?

Amish cultural considerations:

  • don't typically use modern technology such as cars or TV
  • speak simply
  • Christian religion
    • funerals are at home with plain caskets



What are some cultural considerations for Arab Americans?

Arab American cultural considerations:

  • men are more likely to be the main decision-makers


What conditions are Asian Americans more at risk for?

Asian Americans are more at risk for:

  • lactose intolerance
  • thalassemia (blood disorder that causes anemia)


What are some cultural considerations for Asian Americans?

Asian American cultural considerations:

  • may not make eye contact or turn head when listening
  • may not report pain; nurse will need to ask multiple times if client needs pain meds
  • may not want to be touched


What is yin-yang in regards to eating?

Yin-yang is a Chinese practice where:

  • cold foods are eaten for a fever
  • hot foods for a non-fever


What conditions are Hispanic and Latino more at risk for?

Hispanic and Latino are more at risk for:

  • lactose intolerance
  • parasite infection (due to coming from a tropical region)


What language and religion is common with Hispanic and Latino?

Hispanic and Latino:

  • many only speak Spanish

What is the "evil eye"?

Evil eye is a cultural belief that starring at a client can cause a curse or sickness.



What are some cultural considerations for Hispanic and Latino?

Hispanic and Latino cultural considerations:

  • very likely to show emotion and have a big family
  • may look at disease as punishment for sins
  • don't mind being touched in social situations such as hugs


What conditions are most common for the Indigenous people of North America?

Indigenous people of North America are more at risk for:

  • alcohol abuse
  • tuberculosis
  • diabetes mellitus
  • lactose intolerance
  • gallbladder disease


What are some cultural considerations for the Indigenous people of North America?

Indigenous people of North America cultural considerations:

  • may be very quiet


What are some religious considerations for Catholicism?

Catholic religion considerations:

  • Priest is the minister of the Church
  • fast during Lent (no meat on Fridays, but fish is OK)


What are some religious considerations for Jehovah's Witness?

Jehovah's Witness religious considerations:

  • can't receive blood transfusions or blood products
  • donated organs must be washed with a non-blood substance before being used
  • need Rabbi's approval before being an organ donor


What are some religious considerations for Islam?

Islam religious considerations:

  • many are Arab American
  • Allah is the name of God
  • may face mecca to pray (west or southwest)
  • fast during Ramadan (fast during the day, but will break fast with figs and eat after sundown)
    • kids and menstruating or breastfeeding women don't fast


What are the religious considerations for Jewish or Judaism?

Jewish or Judaism religious considerations:

  • Rabbi is the religious leader for the Synagogue
  • is a religion and ethnic group
  • food is Kosher (fit to eat)
  • can't eat meat and milk together
  • no pork or shellfish
  • fish is OK to eat
  • don't touch their food, plate or utensils
  • embalming and cremation is forbidden



What are some religious considerations for Hinduism?

Hinduism religious considerations:

  • many people from India are Hindus
  • many are vegetarians
  • never eat beef
