• it compromises a lack of thyroid hormones, thyroxine and tri-iodothyronine

• cold intolerance

• dry skin
• hoarseness
• lethargy
• ataxia
• cognitive impairment
• menorrhagia
• bradycardia

What are some of the complications?

• myxoedema coma (rare)
• reduced fertility
• PIH
• low birth weight baby
• psychomotor retardation

What is the treatment?

Answer 4

• thyroid hormone replacement

What is the role of the midwife antentally?

Answer 5

• ensure fT4 and TSH are checked at booki and 4-6 weekly after
• encourage compliance with thyroxine supplementation
• refer to consultant led unit if any concerns with growth
• be alert for PIH
• mother suitable for regular, shared antenatal care

What is the midwives role postnatally?

Answer 6

• offer routine care for type of delivery
• breast feeding encouraged
• arrange paed review
• ensure new born blood spot is completed in a timely manner
• ensure dose of thyroxine is reduced back to pre-pregnancy dose
• ensure fT4 and TSH is checked 6 weeks postnatally
