A

Pediatric circuit for 5 to 20kg, adult greater than 20kg

LTV1000 circuit has inline PEEP

Do not use LTV1000 circuit on LTV1200

What are the steps to perform a recruitment manoever?

A

1) Prepare all equipment (kelly clamps)
2) Prepare backup equipment (bvm, opa, ett, bougie, igel, suction - basically if you need to reintubate) - including meds - paralytics, sedatives, pressors
3) Confirm connections are tight in circuit
4) Confirm patient paralyzed and sedated
5) CRM with partner (confirm 40/40 and method to acheive and countdown)
6) Program vent PC mode of 40/0 or 30/10 etc
7) Lengthen Ti (why?) - Better shot at clamping correctly
8) Clamp at inspiratory breath max pressure
9) Hold for 40 seconds - drop breath rate, silence alarm and apnea back up
10) 20 seconds left set up new vent settings
11) Countdown last 5 seconds
12) Release clamp and look for chest rise
13) Check all variables

– in breath rate means patient is responsible for initiaing all breaths to meet minute volume

– in sensitivity mean control mode of ventilation and patient cannot trigger a breath

Describe sensitivity as it relates to the LTV 1000/1200

A

Sensitivity is measured in L/min - low number indicates more sensitive and less volume change required to trigger a patient breath - higher sensitivy means patient has to change more volume in the circuit to initiate a patient breath (9 L/m for example)

You want to match sensitivty to patient condition without being in a state where breaths are accidentally triggered by movement (too sensitive)
