Deaths that must be reported to the coroner

A

Suicides
Cause unknown or no Drs certificate
Unnatural or violent death
Medical or surgical or dental or anesthetiser
Births death or deaths relating to the birth
Institutional deaths - prison, cyfs etc.
Enactment requires an inquest - alcoholism and drug act 1996

Coroner will consider these when deciding to do an inquest

A

Family wishes to hold an inquest
Actions or in actions surrounding the death
Concerns, rumours, suspicions around the death
Elimination of reoccurrence
Cause of death not natural or appears violent

A

Cause or circumstances cannot be clearly identified

Inquests to establish

A

The fact that the person has died - deceased person certificate
The identity of the deceased - deceased person identification form
The cause of death - pathologist report
Where and when the death occurred - police 47 (report to coroner)
The circumstances of the death -statements, job sheets, depositions.
