Sevoflurane pulmonary effects

A

Medium amount (more than N2O) of decreased tidal volume- decreases minute ventilation and increase in end tidal CO2 (same as halothane). Respiratory depressant with very slight tachypnea. Least airway irritation- great for inhalation induction

Sevo CV Effects

A

Dose dependent decrease in myocardial contractility (similar to iso), CO (more than Iso), arterial vasodilation (less than iso) & SVR (less than iso).
Decrease in myocardial contractility causes decrease MVO2 & decrease coronary vascular resistance
No sympathetic nervous system activity
No coronary steal
No incidence of ventricular arrhythmias

Sevo renal effects

A

Oxidatively metabolized to hexafluoroisopropanol formaldehyde, inorganic fluoride & CO2 – can cause renal failure though none have been reported. Belief is that free fluoride may cause inhibition of renal tubular function including chloride transport in ascending limb of the loop of Henle which leads to concentrating effect - just avoid those with renal disease/damage.
