A
LMP Regular? Length of cycle? Dysmennorhoea? (heavy bleeding) Post coital bleeding Intermenstrual bleeding

A
Cervical smear: up to date, results? Previous gynae treatment Contraception? Condom use? Could you currently be pregnant?

A
Pain (groin/testes/penis) Swelling of testes Itch/sore Erythema Lumps, bumps, ulcers? Discharge Dysuria, haematuria, polyuria Systemic (rash, eyes, joints)
