What are bacterial causes of pharyngitis?
GAS
strep - C/G
Neisseria Gon
Mycoplasma
What are Viral causes of pharyngitis?
Adenovirus Enterovirus EBV HSV Rhino Coronovirus infuenza A/B RSV A
What are non infectious causes of Pharyngitis?
GERD post nasal drip allergic rhinitis chronic cough foreign body inhaled irritants Rheumatological issue
Are there any benefit to treating GAS?
Yes
decreases duration of symptoms slightly
dec chance of abscess formation
prevents RF if treated in first 9 days of illness
What is the treatment of GAS pharyngitis?
Pen VK PO for 10 d
Amox tastes better
Pen allergic - erythro, clarithro, azithro
clinda can eradicate chronic carrier
No longer contagious - 24 hours post start of Abx
What bugs are involved in retropharyngeal abscess?
What is the treatment?
Polymicrobial - Staph aureu,strep species and anaerobes
RX- Cetrioaxone and ciproflox (anaerobes) (+/_ Vanco if dont’ respond)
CP of peritonsillar abscess
Usually in adults. Chronic tonsillitis is a risk factor
Fever sre throat dysphagia muffled voice trismus referred ear pain odynophagia
Management of chronic otitis media and effusion
IF affecting hearing/dev: otherwise can wait
T tubes
and may need adenoidectomy
Presentation of Adenovirus URTI
fever rhinno cough exudative pharyngitis conjunctivitis
Bugs causing sinusitis
Strep pneumo-30% (1/4 pen resistant)
H. Flu-20
Moraxella catarrhalis-20
how do you treat sinusitis?
high dose Amoxiclav - 90mg/kg div BID x 10d
