1
Q
Causes of decreased protein production?
A
hepatic failure inflammatory ds (Alb is neg APP) NOT MALNUTRITION (anorexia/malabsorption)
2
Q
Causes of increased loss of protein?
A
- Renal (PLN)
- Intestinal (PLE)
- Third space
- Burn/wound
3
Q
CS of hypoproteinemia
A
- periph limb swelling
- abd distension
- cough/dyspnea (pleural fluid, not as common as ascites)
- decreased appetite
- v/d (loss of oncotic P, osmotic diarrhea)
Can be asymptomatic
4
Q
General categories of proteinemia affecting young and older pts
A
young - congenital v. infectious
old - inflammatory v. neoplasia v. metabolic
5
Q
Localizing clinical signs
- PU/PD indicates?
- v/d?
- inc. stool ?
A
- PU/PD indicates renal, liver ds.
- v/d (intestinal, unless HGE not significant loss)
- inc. stool indicates EPI
6
Q
What changes to EENT in PE may indicate a hypoproteinemia is present?
A
retinal detachment/tortuous vessels - hypertension (renal)
oral ulcers/uremic breath (renal, progerssing tubular ds)
7
Q
hypoalbumin v. hypoglobulin?
A
albumin (dec. prod, PLN, PLE, 3rd space)
v.
globulin (PLE, Blood loss, fail passive transfer, combined immunodeficiency)
