  1. History of the present illness through OPQRST
  2. Clarify any questions about associated signs and symptoms related to OPQRST
  3. Past medical history (AMPLE)

History of the present illness through OPQRST
O = onset; Did this happen suddenly or over time? (acute/chronic)
P = provocation; Does anything trigger the pain?
Q = quality; Describe the pain.
R = radiation/relief/region; Where is the pain? Does it spread or shoot anywhere else.
S = severity; How bad is the pain on a scale of 1 - 10 w/ 10 being the worst pain you’ve ever felt.
T = time; When did the pain start?

Past medical history (AMPLE)
A = allergies; Is the pt allergic to any medication or food?
M = medication; Is the pt on and compliant w/ any meds?
P = pertinent past medical history; Does the pt have insert correlated condition/disease? Does the pt have a history of insert condition/disease?
L = last oral intake; When did the pt last eat/drink take any meds?
E = events leading to illness; What has the pt done leading up to 911 call.
