1
Q

Traditional Vs. Non-Traditional Fittings

A

Trad - HL on both sides is similar, both ears can accommodate “traditional hearing aids”
Non-trad - non-trad HI required on one or both ears

2
Q

Candidates for Non-Traditional Fittings

A
  1. Severe to profound hearing loss in one ear, normal hearing or aidable loss in the other
  2. Severe to profound hearing loss bilaterally
  3. Outer ear and middle ear deformities
  4. Conductive hearing losses
  5. Significant asymmetrical losses
  6. Sudden sensorineural hearing losses
3
Q

Non-Traditional Amplification Options

A
  1. Cochlear implant bilaterally
  2. Cochlear implant in one ear, hearing instrument in the other
  3. Hybrid device (half cochlear implant, half hearing aid)
  4. CROS/BICROS (Contralateral Routing of Signal)
  5. Bone anchored devices
  6. Middle ear implants
4
Q

Cochlear Implants

A
  • for severe to profound sensorineural loss

- surgically implanted

5
Q

Components of a CI

A
  • transmitter
  • speech processor & mic
  • receiver/stimulator
  • electrode array
6
Q

Candidates for CIs

A
  • bilateral severe-profound SNHL
  • has worn trad HI for up to 6 mo with no benefits
  • family support
  • functioning auditory nerve
  • short term auditory deprivation
  • desire to participate in auditory/verbal society
  • participate in post surgery rehab
7
Q

Hybrid CI

A
  • part HI part CI
  • electrode array is shorter
  • appropriate for: moderate or better low freq HL, profound high freq HL
8
Q

Bimodal Fitting

A
  • HI in one ear, CI in other ear
  • Can be an asymmetrical hearing loss where each ear can benefit from different technology
  • increased speech intelligibility with bimodal in comparison with unilateral CI
9
Q

Contralateral Routing of Signal (CROS)

A
  • appropriate for good hearing on one side and no hearing on other side
  • mic with a transmitter sits on the poorer ear
  • receiver sits on good ear
  • signal transmitted wirelessly from good to bad ear
  • decreases head shadow, improves SNR
10
Q

Bilateral CROS (BiCros)

A
  • hearing loss in one ear, unaidable loss in other ear
  • signal mixed between HI and CROS
  • mic in both hearing aids
  • receiver in aided ear, transmits to unaided ear
11
Q

CROS vs. BiCROS

A
  • both have transmitter on poorer ear
CROS - receiver sits on better ear
BiCROS - HI sits on better ear
12
Q

Bone Anchored Device

A
  1. Sound processor - captures sound via micand coverts to vibrations
  2. Abutment - tranfsers sounds into mechanical vibrators from the sound processor to the implant
  3. Titanium implant - placed in the bone behind the ear, transfers sound vibration directly to the cochlea, bypassing the outer/middle ear
13
Q

BAHA Candidates

A
  • outer eat abnormalities: microtia/atresia
  • discharge
  • formation of the ossicles: ossicular discontinuity
  • single sided deafness
  • large air bone gap (30dB +)
14
Q

Middle Ear Implants

A
  • processor consists of: microphone, amplifier, and battery
  • signal sent to receiver through magnetic symbol
  • receiver produces electrical signal down to the mid ear implant
  • set against the round window of the cochlea where it is able to vibrate the fluid within the inner ear
15
Q

Middle Ear Implant Candidates

A
  • people unable to wear earmolds
  • dysfunctional middle ear systems: conductive HLs
  • better high freq response, speech intelligibility
  • no limitations in hot, dusty, wet environments
