How does the ICF conceptualise Contextual Factors?

A

The ICF conceptualises functioning and disability as a dynamic interaction between a persons health and their contextual factors.

Contextual factors represent the complete background of s persons life.

The ICF therefore recognised the integral role that environmental and personal factors play in influencing the function of people with communication/swallowing disorders.

Why are Environmental and Personal factors important for SP to recognise?

A

Howe identifies 3 reasons:

  1. Therapy aims to improve clients communication in everyday contexts
  2. Communication is collaborative by nature
  3. SP have specialist knowledge about communication and communication disorders that can influence a persons functioning

Describe the levels and domains of the ICF Environmental Factors.

A

There are 2 levels.

  1. Individual- persons immediate environment eg home, school, work
  2. Societal - overarching systems such as services and formal and informal rules of which ICF describes 5 domains.

Domains
1. Products and Technology
Eg voice amplifiers, AAC aids, hearing aids, drugs, hydration tx

  1. Natural Environment
    Eg. Sound, temperature, humidity, lighting
  2. Support and Relationships
    Eg immed family, health carers, health professionals, communication behaviours of other people, animals
  3. Attitudes
    Societal, individuals, awareness
  4. Services, systems and policies
    Support groups, driving instructors specially trained, workplace systems

NB factors are described in relation to the client
Barriers are coded from .0 = no barrier to .4 = a complete Barrier
Facilitators are coded from +0= no facilitator to +4=complete barrier

Describe what is encompassed in the ICF under Personal Factors.

A

Personal Factors are divided into 3 key groups:
1. Potentially changeable
Eg coping styles, personality traits, fitness, lifestyle

  1. More difficult to change
    Gender
  2. Unchangeable
    Age, race, ethnicity
