A

is an ongoing systemic process designed to evaluate and promote excellence in health care to clients.

This includes three things:
1. structure: focuses on the environment where the care is given

  1. process: focuses on how the care was given
  2. outcome: focuses on demonstratable changes in the clients health care status as a result of the nursing care.

Relational continuity: a therapeutic relationship with a practitioner that spans more than 1 episode of care

Informational continuity: it refers to data exchanges between providers and client related to care

Management continuity: the delivery of health care by several providers in a complimentary and timely manner thru shared mgmt plans that are consistent and flexible.
