A

Penicillin/b lactamase inhibitor 3.1 grams q4-6IV, piptazobactam 3.375 grams q6 or combination with flouroquinolone levofloxacon q24 or 3rd chalophorin ceftriaxone 2 grams q24 plus metronidazole 500mg q8

For pseudomonas
Meropenem 500q6, 1gram q8, piptazobactam 4.5q6 or drug combi
Cefepime 2 grams q8 or ceftazidime 2 grams q8 plus metronidazole
